'The phonological abilities of hearing impaired children: Interim results
from the LOCHI study' (Day, et al, 2010) is relevant to this thread. You
will see that ICD occurrence was low compared with other error-types for the
184 children in the study with hearing aids, and the 81 children in the
study with cochlear implants, and that ICD did not occur in the control
sample of 61 children with typical hearing. In the Hearing Aid Group, 3% had
ICD at 3 years, and none had it at 5. In the Cochlear Implant Group, ICD was
not apparent when the children were 3, and again when the children were 5.
LOCHI INTERIM FINDINGS (2010)
Children with hearing aids or cochlear implants:
Have delayed phonological development compared with hearing peers.
Use developmental patterns well beyond expected ages of suppression.
Use the unusual pattern of backing at 3 and 5 years.
Phonological processes negatively impact speech intelligibility at age 5.
There is a significant effect of hearing loss on phoneme accuracy scores for
children with hearing aids at 3 years of age but not at 5 years of age.
There is no significant effect of the age of first fitting of hearing aids
on phoneme accuracy, or the presence of phonological processes at 3 or 5.

Otitis Media with Effusion is associated with backing (Shriberg, et al.,
2003), but not ICD.
