Ann Bosma Smit includes a brief but thought provoking section on speech
assessment and intervention with children with developmental delay (which
she also calls cognitive delay) in her 2004 book *Articulation and Phonology
Resource Guide for School-Age Children and Adults*, pp. 178-181.
Surprisingly, given the book's title, it also includes quite a lot of
information relating to preschoolers.
Barbara Dodd and Sharon Crosbie provide Chapter 13 (pp. 233-243) of Dodd
(2005) - the second edition of Dodd's *Differential Diagnosis and Treatment
of Children with Speech Disorder*.
