I currently have a kiddo (5 yo) who sounds very similar to the transcript here. One thing that was suggested to me was to check for velopharyngeal incompetence. My kiddo did have some incompetence from a recent T&A, but he also has a severe phonological impairment. When he's imitating single mono-syllabic words he's fairly clear. In connected speech or multi-syllabic words, he reverts back to his errors. I've been using Barbara Hodson's Cycles approach with success with him.
